The collaborative consists of four, 1.5-hour, sessions spaced 2 weeks apart (6 weeks total).

Care plans should be more than documents we complete to meet requirements—they can be meaningful reflections of who a person is and what matters most to them. Yet too often, care plans are reduced to clinical language, checkboxes, and generic “I” statements.
In Care Plans Worth Reading, your team will learn to create care plans that tell stories, build trust, and strengthen relationships—while still fully supporting regulatory expectations. Using The Eden Alternative’s GROWTH Model (QAPI) and guided by Artifact of Culture Change #52, you’ll evaluate your care planning process and practice writing care plan stories that deepen connection and honor each Elder’s identity and legacy. This work does more than improve documentation—it helps your community remember why you do what you do. Meaningful care plan stories can support recruitment, retention, and team morale by grounding your work in purpose and genuine relationship.
